E-Forms

Submit your forms online - saving time, and creating efficiencies.

Depending on your security level E-Forms are available for electronic submission directly to the WCB. Once a form is submitted it remains in your history for 90 days and non-submitted forms are stored until you are ready to complete and send.

E-Forms available:

  • WCB Injury Report - save time by filing an injury report online.
  • Worker Earnings Information Form - used to supplement the Worker Earnings Information that is submitted with the WCB Injury Report.
  • Contact Change form - let us know if your address/contact info has changed.
  • Annual Subcontractor Report - to be completed by all employers who use subcontractors.
  • Business Discontinuation Form - notify us of any changes in your business status.
  • Employer Year-End Update - information collected helps us reconcile your account information.
  • Zero Payroll Submission - simple way to report no payroll for a reporting period.

HINT: We have made online injury reporting more convenient in our latest release. You can designate people within your organization to complete the injury report without giving them access to claims data. Multiple users can collaborate on a form - a supervisor can start the form and have payroll add the earnings information without sending paper copies of the form around the building.
